Seems like the medical staff was at fault also for not following up or keeping up with his medicine in take of the blood thinners ?

Please see my separate comment. Sounds like the medical staff did what they were supposed to do. He originally didn’t take the prescribed medication completely according to plan. The modern blood thinners typically aren’t monitored with labs; you expect people to take medication as prescribed.

Totally disagree doctors & medical staffs should always check with their patients in regards to the medications they have prescribed to see if it’s working or needs modification or feed back VC said that he stop taking them after 2 weeks when they were prescribed for 2 months or this is what I heard him say
@King Wong he said that he didn’t keep taking the medication and refill it as prescribed. That’s like someone not finishing a course of antibiotics as prescribed. When there are appointments, patients are asked if they are still taking prescribed medications, but patients are responsible for taking things as prescribed in between them. It is not standard of care nor are there resources to hold the hand of every patient to check in daily or weekly to make sure they’re taking every medication as prescribed. Even if they spoke to him every day, then world you say they should have made sure they actually saw him taking every dose of medication? That would be something like “direct observed therapy,” which is rarely appropriate.
